You are looking the side view of the pelvis of a baby with anal-rectal malformation that we call rectal urethral bulbar fistula. The main and most important part of the operation is to separate the rectum from the posterior urethra making two walls out of one. Closing the fistula and the urethra and then using multiple traction sutures, we put traction on the rectum in order to identify the bands and vessels that hold the rectum up in the pelvis. As we identify them, we carefully burn them, going in a systematic way, gaining a little bit of length every time, until we feel that we have enough length of the rectum to make a connection to the skin. At that point, the operation will be completed.
